Job Description:

To provide case management and counseling services for substance abusing patients and their families to assist them in reducing and eliminating drug and alcohol use and to improve the quality of their lives.

The Counselor working for the Division of Substance Abuse will be responsible for maintaining an assigned caseload of patients in full compliance with State, federal, and program requirements, conduct psychosocial assessments, and interview new patients individually to gather patient psychosocial information.

Responsibilities include but are not limited to the following:

* Provide reassessments twice annually and as needed, develop treatment plans, regularly review patient progress, and provide substance abuse counseling in individual and group formats.

* Provide all case management services, including crisis intervention, implementation of treatment plans, referrals and advocacy, gather patient information concerning patient family members, and perform other related duties as assigned.


Requirements

* Masters preferred.

* Bilingual English/Spanish preferred.
