As part of an interdisciplinary team, performs a variety of tasks to support client's activities of daily living, and teach/model independent living and recovery skills. Provides treatment, rehabilitation, and support services that promote client dignity, independence, individuality, strengths, privacy, and choice through a flexible, individualized approach and in the least restrictive, most natural environment possible.
- Provide individual and group training in independent living skills (i.e., personal hygiene, problem solving, housekeeping, budgeting, grocery shopping, and meal planning and preparation) - Monitor client's behaviors, symptoms, activities, and assisting/facilitating crisis intervention and conflict resolution as appropriate - Supervise client's self-administration of medications and manage/maintain medication supplies and packets - Under supervision, develops, documents and evaluates treatment plans with clients - Oversee the safety and well-being of clients by providing assistance in living area maintenance, such as cleaning, performing apartment, bed, and safety checks - Complete all documentation according to established procedures and timelines - Provide client transportation to facilitate treatment plan implementation - Perform general office and/or program duties, such as new resident orientation, supply ordering, and office area cleaning
